Dental Insurance: We are in-network with most PPO dental plans. For those plans that we are not in-network, we will still file and accept them for you. If you have questions about your coverage before making an appointment please call or email us. It would be our pleasure to assist you and perform a complimentary benefit check for you. Keep in mind that we do not accept HMO, DMO, Medicare or Medicaid plans.

Important for new natients: Shortly after making an appointment, you will receive a text and an email with your appointment date and time. This text/email will also include a link to complete your new patient forms quickly and easily online. Please complete these forms and provide your dental (not medical) insurance information as soon as possible. This allow us to promptly verify your benefits and contact you if there are any issues. Incomplete forms, unconfirmed appointments or missing information about dental insurance at least 48 hours before your appointment time, will result in the appointment time being forfeited and offered to other patients. Keep in mind we do not see kids (under 12 years old).
